Psychological Evaluations
Evaluation fees may vary depending on referral questions and hours of work needed to answer the referral questions. A testing fee agreement and good faith estimate will be provided before the evaluation to ensure clarity in billing and payment expectations.
Evaluation fees include assessment sessions, scoring, report writing, interpretation, recommendations,
and diagnostic research.

Early Admission to School
starting at $850
Cognitive, Educational, and Social Emotional Evaluations necessary for children who are under 5 years old to start kindergarten. Evaluations for early admission to school are not reimbursed by insurance.
Bariatric Presurgical Evaluation
starting at $900
Before weight loss surgery, many insurance companies and doctors require an evaluation to ensure that you know about the procedure and aftercare required. This evaluation includes a clinical interview, brief cognitive evaluation, personality assessment, and eating behaviors assessment.
Specific Learning Disability
starting at $950
A specific learning disability is a processing deficit in reading, writing, or math. The evaluation includes a cognitive evaluation and assessments of reading, writing, or math ability. Evaluations for learning disabilities are not reimbursed by insurance.
Attention Deficit Hyperactivity Disorder (ADHD)
starting at $1300
At Waite PTS, we complete evaluations for children and adults who struggle with inattention/distractability and/or hyperactivity/impulsivity symptoms. Assessments for ADHD include cognitive evaluations that include working memory, structured interviews, rating scales of executive functioning, and assessments of executive functioning. At times, further evaluations for anxiety and depression will also be conducted to ensure that the diagnosis is appropriate.
Autism Spectrum Disorder

At Waite PTS, we complete Autism evaluations for children and adults who report concerns regarding Autism. Assessments for Autism Spectrum Disorder may include cognitive evaluations, structured interviews, behavioral observations, personality assessments, and behavior rating scales.
